Consent forms

The despatch clerk from AYC came to my office at 11.00am. I handed a brown envelope containing the two sets of signed consent forms to him. I told him to pass the envelope to Ms Yap.

Later in the afternoon I messaged Ms Yap via sms to confirm that her dispatcher had indeed collected the forms from me.
